Can a live show be edited and swapped out for the original livestream? If so, will the original comments and url be preserved?

No. It's one of the big bummers of YouTube. You can sort-of edit a show using the YouTube trim tool but all is really good for is cutting off the beginning (like a long countdown)

When do you add transitions? If you add transitions, how do you decide that a transition is needed or not?

Personally I add them as I go and use them sparingly. There needs to be a reason for them. Don't use one just to use one and most importantly don't use anything other than a dissolve or fade to Black without a really good reason. They can just be distracting and look amateurish.
