Maybe I'm so new that this obvious thing escapes me, but how do I make it so that whatever the last version of a given photo in a stack is always placed on the “top” of the stack?

Todd,

If you create a new version while in an Album, that version becomes the Album Pick automatically. It’s like a Stack Pick, except that it’s only the pick (top image) in that album.

If you promote an image to Stack Pick, that is promoted throughout library—that’s why there are different types of picks, and also why it doesn’t happen automatically. To promote to Stack Pick, tap the keyboard shortcut Comand-.
